The Challenge
A large long-term care home was operating in crisis. After failing multiple regulatory standards and placing residents at risk, the organization was placed under external management. Despite three years of third-party oversight, meaningful improvement remained out of reach.
The problems ran deep. Care delivery was inconsistent across units, audits were conducted manually and riddled with gaps, and data could not be trusted to support sound decision-making. Communication between teams was fragmented, leadership lacked a shared framework for accountability, and staff morale had deteriorated significantly. Families were losing confidence in the home's ability to keep their loved ones safe.
The Approach
A comprehensive operational and cultural assessment exposed the root causes: weak governance, unreliable data systems, poorly designed clinical workflows, and leadership behaviours misaligned with organizational values. A structured transformation was designed and implemented:
- A Balanced Scorecard was developed at the organizational level and cascaded to every program and unit, creating shared visibility into priorities and performance
- Frontline staff were trained in Lean management principles, building internal capability for daily problem solving and continuous improvement
- Manual compliance audits were digitized and a new health information system was implemented to improve documentation accuracy, data integrity, and real-time access to clinical information
- Operational and clinical workflows were redesigned to reduce variation, eliminate waste, and strengthen consistency across care areas
- A governance framework was embedded to sustain improvements and reinforce leadership accountability over time
The Outcome
The transformation stabilized operations and restored confidence across the organization. Staff gained clarity in their roles, communication improved at every level, and families began to trust that their loved ones were receiving safe, consistent care.
The home successfully exited external management, achieved more than $3 million in cost savings, and earned national recognition for quality and leading practices in long-term care. It became a reference model for how operational discipline, cultural alignment, and strong governance can reverse a failing organization and build the foundation for lasting excellence.
